Instructions for Filling out China Visa Applications

• Please print clearly in blue or black ink
• Nationality is the country of your passport(U.S.A.)
• Former nationality is the nationality you had when born, only fill this out if you formerly held the passport of any nation other then the U.S.A.
• Occupation and name of company: If retired simply write in "Retired"
• If your passport is blue or green, the Passport type is Ordinary. If you have a diplomatic or official government passport you know who you are.
• Passport number can be found in the upper right hand corner
• The expiration date is clearly marked in the mid right side
• Issued by: can be found on the bottom listed as AUTHORITY:
  Example-Authority: Passport Agency Los Angeles
• Intended date of entry: The date your tour departs the U.S.A
• BE SURE TO SIGN AND DATE THE BOTTOM
• BE SURE TO ATTACH a headshot (approx 2"x2")photo to the upper right hand corner (NOTE: only standard passport pictures on a white background will be accepted by the Chinece Consulate)

• Please bring in your passport with the visa application completed to our office or send the documents either FedEx or some sort of secured mail
• Allpassports must be processed at the chinese consulate and takes 5-7 working days. They can be expedited for an additional $25 (48 hours)
